- Fill out and sign the “Foundations
Skills for the Performance
Team Group Class Application
and Agreement.” Be
sure to fill in the
start date and time
of the class you want.
If you are not certain,
please contact us for
clarification.
- Mail or fax vaccination
records. Fax number
is 562-423-8993.
- Enclose the
application and a check,
payable to “DogPACT” for
$85.00, and mail to:

please call the class

press option 3 for
a class update.
Items to bring!
- A few different yummy treats
- 1-2 favorite toys and a good, rope tug-toy
- A light drag line, 6-8 feet long

you have questions about their
services or for last-minute
directions if you get lost.

Classes will be held in the
parking lot behind the shopping
center. You can park in the
main parking lot in front of
the Paw Shoppe, and walk through
the store, or you can park in
back where the class will be
held. The driveway is the service
entrance off Palo Verde at Belen
(on the east side of Palo Verde).
