Application

Class Enrollment Instructions

1. Fill out and sign the “Pet Manners 1 Group Class Application and Agreement.” Be sure to fill in time of the class you want if it is not already filled in for you. If you are not certain, please contact us for clarification.
2. Fill out the “Training Goals Questionnaire.”
3. Mail or fax vaccination records. Fax number is 562-423-8993.
4. Enclose  the application, questionnaire, and a check, payable to “DogPACT” for $125.00, and mail to:

The First Night of Class

• No dogs, please! Your instructor will go over the goals of the class, introduce clicker training, and hand out materials, etc., which is much easier if you aren’t wrangling a dog at the same time.
• Bring a chair or blanket to sit on. Class is held outdoors and depending upon the weather may be chilly so please dress appropriately.
• In case of rainy weather, please call the class hotline at 562-423-0793, and press option 3 for a class update.

Note: Please call the Paw Shoppe only if you have questions about their services or for last-minute directions if you get lost. Please call DogPACT at 562-423-0793 (press option 1) with any other questions about the class.

Classes will be held in the parking lot behind the shopping center. You can park in the main parking lot in front of the Paw Shoppe, and walk through the store, or you can park in back where the class will be held. The driveway is the service entrance off Palo Verde at Belen (on the east side of Palo Verde).
