Class Application

Enrollment Instructions

1. Fill out and sign the “Pet Manners 2 Group Class Application and Agreement.” Be sure to fill in the start date and time of the class you want. If you are not certain, please contact us for clarification.
2. Mail or fax vaccination records. Fax number is 562-423-8993.
3. Enclose the application and a check, payable to “DogPACT” for $110.00, and mail to:

Rainy Weather

In case of rainy weather, please call the class hotline at 562-423-0793, and press option 3 for a class update.

Items to bring

• A variety or small-sized treats
• Standard 4’-6’ leash and a long 12’-15’ line
• A couple of your dog’s favorite toys
• Towel or mat for the ground and teaching “Settle”
