Class Application

Class Enrollment Instructions

1. Fill out and sign the “Real-Life Pet Manners Group Class Application and Agreement.” Be sure to fill in the start date and time of the class you want. If you are not certain, please contact us for clarification.
2. Mail or fax vaccination records. Fax number is 562-423-8993.


3. Enclose the application and a check, payable to “DogPACT” for $85.00, and mail to:

The First Night of Class

• Meet in the parking lot just north of the Westmisnter Veterinary Group building
• In case of rainy weather, please call the class hotline at 562-423-0793, and press option 3 for a class update.

Note: For last-minute directions if you get lost. Please call DogPACT at 562-423-0793 (press option 1) with any other questions about the class.
